Follow up "demographic/invoicing" form for ticket holders #87

Please, put here possible fields for the google form.

General

  1. ticket number
  2. email

    Demographics

  3. Coming from
  4. Gender

    Invoice

  5. Official Company name
  6. Address
  7. VAT Reg. Number (if available)
  8. Reg. Office (if available)

    Other

    • T-Shirt size
    • Interest for light lunch
    • Interest for side activities (spouses)
    • Interest for vacation suggestions

Added the following fields:

  1. Origin
  2. Gender
  3. Interest for side activities (spouses)
  4. Interest for vacation suggestions

In the 'Interest for light lunch' there should also be a small text field for them to fill out any preferences. Remember the 'Vegetarian/Kosher/Halal' thing @pagojo was talking about.

Guys I am a bit confused which is which. Light lunch is the catering thing we are arranging or is it the sandwiches. In case it is the first I wouldn't call it "light" it is normal lunch.

I think we decided the catering will do a deal with Eventora and sell "food tickets" through there. They only thing for us to do is communicate this to our delegates.

Unless, you want to add a question in the current form to give the catering a first indication of how many will be interested. So we can add something like "There is a snack bar in the premises that can satisfy some basic nutrional needs. But the organizers would like to give you the option to have a proper meal (payable by you, the attendee). There will be four different combinations to choose from. Would you be interested in pre-ordering a meal?"

Also, if we put a gender we have to be creative and not limit it to Male, Female. IMHO I am struggling to remember a conf a month ago that had a form with crazy gender options (alien etc.) to show their liberal views on the subject.

Also, about the T-Shirt I think I can see a general consensus that we will have one (and are not considering alternatives e.g. cup/baseball hat etc) so if we put the question up we are bound to provide one. It will be a bummer not to. Just so you know.

but it is just a spreadsheet so the comments can't be on the actual form

  1. No and we are lucky we found something close to Ruby
  2. If we change the title people might skip it because they might think it is only for proposing new stuff/suggestions

  4. No, we cannot do that. There are certain fields that need to be present in the invoice (through our app) and I cannot fill them/split them manually. As you might have noticed the invoice fields are optional, so the people who don't need it, will skip it. I decided not to do it on an e-mail to e-mail basis (although the demand might be low), I wanna cover my ass in case many people start asking for invoices i.e. I want to automate it.

@nolamesa helped me shape up the form a bit and add some witty options (thanx!). Since none of you made any remarks about additional questions: @nikosd I think it is ready to go! Do with it as you please

Looks good @apantsiop! Btw, did you check that the responses are stored somewhere? Are they going to show up in excel sheet you sent us?
