Received a reply from Spotify:

Dear genderswap-fm team,

Thank you for submitting genderswap-fm to Spotify for review. This is a notification that your submission was not approved for the following reason(s):

  • Modifies any Spotify visual content, e.g. cropping album artwork, overlaying images on album artwork, placing their brand/logo on album artwork (Design Guidelines)
  • Metadata, cover art and audio content must be accompanied by a Spotify logo and a link back to the applicable artist, album, track, or playlist on the Spotify Service (Design Guidelines)
  • You must have a mechanism for users to disconnect from the app If you still need to extend the quota of your app, please address the issue(s) described above and submit a new quota extension request.

Before submitting applications for review, please ensure compliance with the Spotify Developer Terms. Addressing the above issues does not guarantee that Spotify will upgrade your application on resubmission.

I'm contesting this because the app doesn't actually violate any of these reasons.
