Reject push if change-id missing #599

Require a change-id line in a commit (reject it if it's missing), like the 
requirement for signed-off-by.

This will prevent issues and pain when a user forgets to add the change-id when 
using programs that don't trigger the gerrit commit hook like eGit, Gitx etc...
