Purpose of code changes on this branch:
this commits fix compilation under mingw. It also addresses few (but not 
necessarily all) runtime issues in tcmalloc windows .dll built this way.

When reviewing my code changes, please focus on:

Looks solid. Go ahead and merge this into the main trunk. Just need to make 
sure we capture the new setup process which requires the user to run autogen.sh 
(and probably 'make configure'). Maintainers for projects like FreeBSD will 
need this information.
