Settings to add before & after caption div markup #80

Implementation of settings to add markup before and after the 'carousel-caption' div. This allows much more flexibility when creating carousels with custom width/position captions.

I haven't done the translations - I'm not familiar with the format. If I have time I'll look into that too.
