Problem with the SmartApp starting on Samsung Phone #4

When I click "OmniPro Integration" on my phone it takes me to Settings with two fields "Assign a name" & "Set for specific mode(s)". I have tried many different entries and get the same results. "We're having trouble connecting. Check your network connection and try again." I don't see any IP or Port settings. Any suggestions are appreciated. I tried another SmartApp and had no issues..
