This converts our EBP issue templates to use the new GitHub Issue Forms syntax. It doesn't change much of the template structure itself, though does make a few modifications to language and directions.
My hope is that this will make it more likely for users to provide the right context needed to address requests, fix bugs, etc. I hope it will also make it less likely that users just delete the provided markdown and write whatever they want 🙃
