License header within the source code files #45

We should add license headers to our code files. Furthermore we could use this to add an AUTHORS file and add the full license text in a LICENSE file.

[ ] All files have a license header
[ ] An AUTHORS file exist in the project root folder
[ ] A LICENSE file exist in the project root folder
