Update READMEs for all exercises to remove boilerplate #115

Addresses issue #111
Exercises now use their specific name instead of what was created by the generator

NOTES:

some auto-formatting was done to the Markdown files by Atom, but none of it seemed harmful to me

Outstanding.

SML must be the most privileged track on Exercism with more active contributers than students. :-D

I don't mind the auto-formatting being a part of the commit, and I like the homogeneity, but I don't know if we can expect it to be consistent over time if you're the only one who runs it.

Would it make sense to have a commit hook across all language tracks that runs a Markdown linter? I'll take this question elsewhere.
