Add participant instructions - internal server error

Describe the bug When I try to add Participant instructions, I get an internal server error and cannot access my Panl questionnaire item anymore.

To Reproduce Steps to reproduce the behavior:

1. Go to a project
2. Click on 'Add item'
3. Select Panl Questionnaire
4. Got to workflow
5. In the library click on Add below 'participant instructions'
6. See error

Expected behavior An added participant instructions tasks to the workflow.
