missing values in PDF #108

On detached - these fields are in the HTML form, but missing from PDF when entered:

Buyer name (row 2 in contract)
Seller name (row 2 in contract)
Property address (row 4)
Checkmark for “seller agrees to provide buyer with home warranty at closing (row 25)
Offer deadline (row 110)

Additionally, these checkboxes are not the html form, but should always be checked:

this isn’t in the form, but ALWAYS check the 2nd checkbox “Purhcase Price, OR [] a total oc” (row 28)
Mortgage contincengcy percent (2nd checkbox on row 35)

confirmed by eric. spot checked firefox and preview. looks good.
