Minor typo fixes #158

Hi,

Here are a handful of minor typo fixes:

input names in table (P_UP -> P1_UP / COIN2_P2 -> COIN_P2) (reformatted the table to be more clean, hopefully that won't mess anything up with LATEX)
missing comma in init_vars.c
comment alignment in cps1-2.lk

Thank you for making this book better! Small nit: Try to send PL with individual commits, they are easier for me to review (I need to make sure the layout of the page flow is not affected).
