fix menu, show unlogged tasks

• display individual tasks if the user has a log for them or not. Also:
  • display checkbox state (completed / not completed)
  • display radio button state (confidence level 1, 2, or 3)
  • add task id to a hidden form field's value attribute (post task to database in a hacky way)
• make the menu work to:
  • bring the user to the parent page from each task
  • hide the back button on the dashboard
