Enhance consent information #117

Add to Individual Consent:

Info on Incidental findings, e.g. "data may be reused in research with a chance of IF"
Allow linkage (ie. bringing information from different sources together about the same person)
Allow reidentification (implicit: recontact)
Recontact method (link to recontact info document)
Restrictions from patient side (rare): all DUO terms instead of subclass

Informed Consent:

Restrictions from institute side (common): all DUO terms instead of subclass
