Add a progress bar? #18

Not super important, but it would be nice to add a progress bar to see how far along things are. It looks like there's a way to do it, but it may not be super straightforward.

Currently we do bootstrapping w/ the boot package, and I'm not sure it's supported. If we did bootstrapping manually, then do parallel processing also manually, it could probably be added. e.g., furrr package has it built in.

If furrr is used with the newer boot.modmed.mlm.custom, it ought to work: b9da063a1c0cffd242646b2f82e3a817525f51bc
