Vision Care Providers (eg. VSP) #164

It would be good to have this health provider added to the system.

Hey @ebdavison Yeah I'd love to support Vision care providers. So far I haven't been able to find FHIR API endpoints for them, but they are covered under the 21st Century Cures Act.

The only thing I've been able to find so far is https://confluence.hl7.org/pages/viewpage.action?pageId=82914199
