It would be great to integrate with or have internally a journaling system, where a patient can record observations and notes into a database. It would also been nice to add/tag datapoints like health records along with it.

I wonder if this could be merged with #260? Patient notes on encounters sound like the first part of what you're suggesting, and patient-initiated provider-less encounters sound like the second.
