Added timestamps to events
joined Scene and SimpleManager with GameHandler, removing need for double implementations of destroy methods/ui constructors
reduce throwing of RejectedExecutionException from ManagedList

- replaced with accessor methods, instances were privated and deprecated for "removal" in a later FastJ release
ensure default fps will be 60 if non-headless environment's monitor refresh rate is 0 or undefined (headless exception is thrown)
