HTML content in no namespace is ignored #349

The HTML to EPUB 3 conversion assumes that the input HTML content declares the 
standard HTML namespace `http://www.w3.org/1999/xhtml`, which is recommended 
but optional in the HTML syntax.

An HTML document with no `xmlns` attribute on the root `html` tag will fail the 
conversion.
