Generated EPUB 3 nav toc is incomplete #352

With the html-to-epub3 conversion, the toc nav of the result EPUB 3 is 
sometimes incomplete.

A sample file is the "Accessible Publishing – Best Practice Guidelines for 
Publishers" document from WIPO/DAISY/EDItEUR
