layout issue on ActivateUser JSP's #418

The User Activation Entercode JSP and the User Activation agreement JSP 
contains two forms, because the submit and cancel actions needs a different 
form. The current HTML of those two JSP's result in the buttons being put on 
multiple lines.

This makes the UI a bit inconsistent. Therefore an enhancement to display the 
form elements inline.
