Update Form 99 to allow for a PDF attachment

Business Reason

As a Committee Administrator, I will be able to attach a pdf file to my Form 99 so that I can submit an updated Loan Agreement or other document along with my Miscellaneous Text.

Acceptance criteria

Given the user is on the Reports page

When I go to create a new report and create a Form 99

then I will be allowed to upload a pdf document

gregg.moreland commented: [~accountid:61b0b42cd5986c006a9e1c94] Initial draft of ticket to add PDF upload to the front end; Additional tickets will probably be needed to attach PDF to the Form 99 when it is submitted.

[~accountid:557058:3e765e5c-5614-4ef8-975a-6f5202ed75a9] From the Format 8.5 specs, there is a new field (PDF ATTACHMENT) on the Form 99 spec that is required if a pdf is attached. We will probably need other ticket(s) for the backend to accept an attachment and submit the Form 99 with an attachment.
