Allow to create new headers after removing all #17

When all headers are removed via Trash button, there is no way to define new headers

Fixed in https://github.com/feedhenry-templates/fh-api-mapper/pull/22 via CSS - no delete button on first header. Note if key and value are empty, no headers get sent :-)
