Same issue on Big Sur 11.2.3 and VSCode 1.55.

This extension looks abandoned. If someone else is stuck without definitions and outlines like I was, uninstalling this extension and installing PHP Intelephense got me all the features back. If I get updates on the ongoing issue with this extension I'll delete this comment, but in the meantime it may restore full performance in the short term to some devs out there.
