add location to output #16

write the process to include outputting based on location. start with just state variation. write it to only look at states where they have done business.

have added state locations; closing this for now. statewide is all we need for the current idea. there is an issue w/ statewide, as it attempts to only add states for those states in which the FI has applications. however, it determines this from only a certain year and therefore it might not hit other states. again, this will work for the current idea, but needs to be updated if it were going to be something much more robust.
