pfnCrashCallback2 is missing in tagCR_INSTALL_INFOW #211

Hello,

This is an awesome library. Thanks to you guys, I saved a lot of time. I'm not 
sure it is intended or not, but I found some bugs in crInstall function.

1. pfnCrashCallback2 is missing in tagCR_INSTALL_INFOW
2. Although tagCR_INSTALL_INFOA has pfnCrashCallback2 member, it is not being 
set in crInstall function.

I hope you can fix this issue soon. Or,if you join me to this project, I can 
commit the changes by myself.
