Add option to disable prefix entirely #9

As the title says, it'd be nice if the plugin had a way to disable the filename prefix entirely.

In the Custom PrintJob Prefix Dialog, there is a checkbox "Add prefix to job name". Unchecking that disables the prefix. It mirrors a similar checkbox in the General preferences.
