Filter for "Fully Open" metrics #39

Allow a filter to restrict output reports to fully Open metrics so that we can support Non Commercial datafeeds but still promote and allow a fully Open output for those parties that need it.

Requires

a checkbox on the input page for "restrict to Fully Open metrics".
plugin registration specifies a flag to specify if its metrics are Fully Open (it is easier to do this at the plugin level than the metric level?)
only call fully Open plugins when the check is checked
