Remove dupes / adjust names #133

There were several duplicates. Also a few names/orgs were misspelled/misattributed.

I have not adjusted the S/N column: it would make the diff unreadable. Needs to happen in another commit

Thank you @ribasushi I am currently updating this record now. I have asked other Core Devs to also take a look and make sure everything looks good from their end.
