MSVC compatibility #219

Instantbird wished to upgrade to libqq from the old libpurple plug-in for QQ, 
which was done at https://bugzilla.instantbird.org/show_bug.cgi?id=1021

Part of this process was updating some of our MSVC hacks into the libqq source, 
I'd like to upstream those changes, if possible.

I believe all the changes from that bug are applicable except the first one two 
(where we replace #if _WIN32 with #if 0). I've modified the diff I created for 
that bug and attached it.
