Add per-hospital random intercept #109

The NELA risk calculator adds a 'random intercept term for each hospital to account for any lack of independence in the data owing to patients being clustered within hospitals'. We don't include this in our implementation of the current model.

Perhaps we should include it in the novel model too?

This isn't used in prospective prediction (in the NELA calculator web app) so I think is just required during model fitting, therefore could be implemented even given our anonymised hospital IDs.
