This PR covers the last part of #239:
Add accelerator git submodule attachment to the CFI repo for each of the extant TF child sub modules.
Where Terraform code which was previous migrated to such repos was completed, so that is removed from the respective accelerator repo and the submodule attached at the same point in its place. This has only happened to Azure Postgres as per the PR diff.
