add initial Azure support #33

Adding initial support for Microsoft Azure. Does not include partition and LVM support in order to avoid LVM naming conflicts with cloned VMs. Will want to eventually support LVM on data disks as part of Azure recipe.

Would it make sense to create separate Markdown files for each IaaS provider? Seems like the instructions may be different enough that it would make them easier to follow.

@afeld I agree! Also wasn't sure whether or not you wanted to create separate directory structures for each provider. Created one for Azure but we can move things around as you see fit.
