Clean up trace roots in @fixieai/sdk #401

This makes the FixieRequestWrapper the top-level renderable for Fixie agent requests which structures the OpenTelemetry traces in a more straightforward way.

Additionally, the instrumentation configuration now automatically propagates any ai.fixie.* baggage properties to logs and spans.
