Adding or updating content now more closely mirrors in-app behaviour:
when adding an entry for a non-default locale, a placeholder entry for the default locale is always created
trying to update a missing entry will try and create a new entry instead honouring the entryId passed to it
There is also now the option to pass an optional env or locale to content methods to override these settings at a query level instead of updating them for the app context.
