Fixed some build issues, did some cleanup, and fixed a few FM voice TL issues #1

See 12af013b4ddbd15fa5b180576cff8e329d279ac7's description for an explanation of the TL high bit issue, also mentioned at flamewing/mdtools#1.

I also added a fix that preserves a certain behaviour of SMPS 68k songs/sound effects in SMPS Z80 drivers, which wasn't previously being accounted for.

There were also a few issues that kept the driver from building properly, when I installed it in an S&K disassembly, which I've tried to address as well.
