Fix more note-rest-duration-duration bugs #14

These fix Carnival Night Zone 1 & 2, and the Sonic 3C credits music.

CNZ is special, because its bugs aren't note-rest-duration-duration at all: they're just rest-duration-duration as they're the first bytes of the track, causing uninitialised notes to play.
