HTML indexer should also index .htm files #46

Currently only files ending in .html are indexed.

At the moment we determine file types just by looking at extensions, so the 
easy way
to  do this is just to add an .htm format. In the long run we could adopt 
something
richer - see http://code.google.com/p/flaxcode/wiki/FileTypeRepresentation, but 
I'm
not sure whether we want to spend the time doing that now?

For the time being I'll simply add .htm as a format, defaulting to IFilter 
filtering
on windows, and we can create a new issue if and when we decide we want 
something more.
