As a temp outfitter, I would like to be aware that I need to provide an Acknowledgement of Risk Form only if I require my clients to sign one. #718

Change wording to make it clear that this is required if they require their clients to sign one. Keep it optional for organizations that don’t offer this to their clients.

Change text "If you plan to use an acknowledgment of risk form, attach a copy." to "If your clients are required to sign an acknowledgment of risk form, you will need to provide a copy before your application can be approved."
