Observability page is missing the Hystrix dashboard even though the Wiki says it is there #94

The monitoring dashboard accessed via http://localhost:9999/admin/dashboard is broken. Functionality and description in Wiki pages are not consistent.

Fixed this to now show both the Hystrix Dashboard and the FSM View on the same monitoring web app. Wiki documentation updated to reflect this.
