No coffeescript runtime #107

Currently fbp-protocol-client requires a CoffeeScript loader registered into Node.js to run. This PR changes to provide built JS versions in the NPM package

I don't see much of a point in moving the source files, but whatever.

@jonnor it was mostly for cleaner handling of the helpers files
