Please Create Changelog #124

Could you please create a changelog? It is difficult to identify what changed in the this library for every release. Specifically, it is difficult to identify the breaking changes from 2.x -> 3.x

Breaking changes are following:

Drop Node.js 4 support
Add winston 3.x support, drop winston 2.x support
