GH-170: Fixed optional focus indicator in modals. #174

Fixes optional focus indicator to work with modals, and also to track changes in focus that aren't made by the Gamepad Navigator, such as hitting tab or using keyboard arrows.

Moderately severe scare here with #175, but that's fixed by this pull as well.
