Describe the bug

I crawled the handbook and found a couple minor errors. Not a big deal, but there are tools to catch this.

Thanks @mgifford. Automated accessibility testing is a good idea. I think others at the IDRC have been experimenting with possible CI solutions. I'll leave it to them to comment on this specifically.

In the meantime, I'll take a look at the issues axe has identified.
