Motor problems preventing sinusoidal commutation #193

There are a three problems currently preventing enabling sinusoidal commutation 
on ATRIAS.

1) All leg motors report inconsistent encoder index pulses
2) Right B motor reports ~1900 encoder counts per revolution instead of 3500
3) Measured phase according to amplifier jumps by ~100 degrees while motor is 
rotating
