add location form updates #25

Add a location/u-map should have additional fields to add that are a closer match to the fields in the xls. (there will be a text blurb added here for ensuring that the user takes full responsibility that their location entry is correct. Here are the current fields that should exist:

text blurb
name of organization
Address
description
URL
FB + Twitter Link
Phone Number

From #26: Add a location/umap form should have a cancel button. Add a location should also be a popup dialog. Also u-map and add a location buttons should be moved to overlay on the top of the map to the left of the map and satellite buttons provided by google maps.

Not sure about moving the u-map and about buttons now that the map is width:100%.

Ignore the moving the u-map buttons bit at the end. I misread that part. Totally doable.
