ACQ/ERM - Derived table agreements_package_content_item updated #837

Closes #836

The derived table agreements_package_content_item needed an update. The relationship between the title_instance and identifier_occurrence tables has changed.

The PR is blocking the development for #840 and needs a review before we can start with the next update for the table.
