Notes from SW Local Care Network task and finish session 16/04/15 version 2 Bringing asset mapping and mental health for long term conditions together
What do we want/need to achieve? To develop an asset map

DEFINING A COMMUNITY ASSET
• a description is that this is a directory

• a community asset can span across boundaries; private, public and 3rd sector
FOCUS
a) to bring asset mapping starting with mental health for LTC together but then progressing to beyond
b) to find out what kind of assets, maps exists in SW Locality and/or Lambeth, with a view to tweak and combine into one place
c) to sign post and identify where the gaps are
d) to find the information and to make sure its used by all people and providers across health, social care and the voluntary sector
e) to develop ways to obtain information that can be useful. Look at information use and distribution and use from a practical perspective
f) to recognise that this is a moving process
g) linking in access to the benefits advisor for advice on benefit, occupation, income, employment,
ROLE OF THE SWLCN WILL BE • responsible for completing the asset map • to support and promote its/their usefulness • to raise awareness, identifying who needs to be aware. For example, organisations on the directory, individuals • to be in a useful format. For example in e-version with a word search facility. Good examples include the SLIC/older people and Age UK older peoples’ directory • to link in and use a platform that already exists. For example Lambeth CCG, Mental Health, Lambeth local authority.
OVERALL AIMS • To develop an action plan to do an asset map. To do this we need to o identify who will do the work o identify the budget available o put time frames against each task o ensure every task/action has an identified person against it
• that the information available improves the wellbeing of the general public • that the information supports and facilitates the health, social care and the VSO • to establish whether there is a place for a paper directory • to establish links with other websites and build upon what already exists • to identify a website where the asset map is to be hosted.

Overall Aim: To • To create an Asset map. • Deepen our understanding of what exists in the South West Locality • To understand the gap analysis – can it be plugged? • To capture all services and groups beyond Lambeth. • Allows relationships to be formed with other Organisations. • To have a list of Organisations/ directory. • To ensure the service is accessed frequently. • To ensure what exists is maximised and sufficient for the people using it and is being made use of. • To ensure there is a dialogue prior and ongoing with Organisations, and that they understand why they are in the Directory. • To ensure that there is a community contract. Why is this important?
What is the issue or challenge we are addressing? • To complete our health needs assessment understanding • Live sharing of information e.g. GP online asking questions as a way to connect people, a process. Build into other systems e.g. Emis • To empower people to be informed agents in their own care/life. • A lot of needs will fall out the public health information. • People and patients do not know what’s out there yet. They want more GPs/ Practices

• Start by not mapping the ‘world,
• Prioritise and maps a part of what is in the South West Locality e.g. isolation hub, obesity and violence. • Link in Local Authority colleagues and their strategy ‘living healthy for longer’ – certitude, Independent Living, partnership. • Mosaic/ Living well Partnership • To map what is fully functional to people and to ensure its usefulness to citizens. • Develop a directory on Health. • To define the area in which we want a Directory for – on wellbeing for people who are not too ill. • To use, access and implement the directories that’s already out there. • Get the information. • To enable a cultural change to Service Providers. • Focus on Mental Health Mapping. • Get experts in on MIT to look at diabetics. • Funding and identifying the ‘doing’, getting it up and running while maintaining it. • LCN role to ensure that it is working and feeding into asset mapping for Mental Health, patients, Pharmacies and GPs. • Stacey Hemphill will take Mental Health to LWN for continued ownership. • To have a Mental Health and Wellbeing DIV.

• What about preventative wellbeing – not a condition.
What is the next step…?
• Identification of members for task – finish group
• This will inform the South West Location network development plans
• Engaging with other Partners as part of identification, so that they are involved or made aware of the community asset.

Many thanks for taking the time to see me, particularly at the recommendation of Nicola Kingston, it was a pleasure meeting you and Giles on the 22/04 to talk about mapping, in particular the work that you are engaged with in mapping the mappers across social and health care in Lambeth. I mentioned that the healthcare, South West Lambeth local area has developed a local care network composed of colleagues across health; primary, secondary, Mental Health, children and adults, social care and the voluntary sector organisations. The development of the local care network is part of the wider transformational programme in primary care where Localities, in which there are 3 in Lambeth: N, SE and SW, each come together separately, to work at a larger scale to enable integration with all key providers, support general practices to promote new and different ways of working that is joined up and co-ordinated for its local population in the community/neighbourhood.
I found our discussion in better understanding community asset mapping, what activities is available locally within the neighbourhood, and how do you reach those communities and making them known, most interesting and stimulating. This links in very much with the work that the SW Local care network has been tasked to achieve, looking initially at developing a community asset map for people with a Mental Health illness with a long term condition. The attached notes captures the groups initial thoughts/discussions on this. We both felt that there is scope and an opportunity for collaborative approach and your experience and expertise would be invaluable, particularly as we progress with this project in the SW Local care network.

Another user could be the local GP or their patient participation group. We could have a real life example to work with very soon. They want to be able to direct people to local 'wellbeing' places and activities eg community gardens, reading group, walking group
