Merge CD12 into root and remove CD12 directory

This is a monster PR, I realize. I've removed the CD12 directory...

There were minor differences between the files in cd12 and the root

• Some files in root were missing Microsoft copyright header and vice versa. These were resolved to maximize the use of the copyright header.
• There were some discrepancies in redactions between root and CD12. These were resolved to minimize redactions.
• Some files did not exist in either CD12 or root. These were resolved by only adding files to the root and never deleting any files.
  • This includes kauai/obj files. Which aren't strictly needed but added for to remain consistent.
• Where there are only formatting / whitespace differences, the root version is maintained as is.
