DOC: Provide PR template #337

Description

This PR introduces a Markdown template that can be used in future PRs to the BDSS.

Including it in the .github directory will make it discoverable by GitHub: https://help.github.com/en/github/building-a-strong-community/creating-a-pull-request-template-for-your-repository

Additional reviewer instructions for developers are also provided

Should the PR review be in a specific format?

Good question - I don't think so, and in any case I don't think GitHub provides a way to contribute a template for a PR review, so it would just be a pain to have to copy + paste each time.
